Passport Application
Important:
• Applicants are required to appear in person when lodging the application;
• Application forms can be obtained from the High Commission or by written request, enclosing a prepaid self-addressed envelope (A4 or A5);
• All photocopies must be authenticated as true copies of the original documents by the Commissioner of Oath at the High Commission who also signs as recommender on behalf of Zambians living in Malaysia and countries of extra accreditation;
• The Passports applied for, through the High Commission, are issued in Lusaka, Zambia. Therefore, all questions must be correctly answered in order to avoid delays. The minimum processing time for new passports is sixty (60) days.
• Passport size photos (64mm x 51mm or 51mm x 38mm) should be taken against a white background (no braids, hair extensions, hair bands, earrings or necklaces)
Requirements
a. Adults
• Duly completed Forms
• Two (2) recent passport size photographs,
• Photocopy of National Registration Card (NRC)
• Photocopy of Birth certificate where applicable
b. Applicants under the age of sixteen (16)
• A duly completed Form
• Two (2) recent passport size photographs
• Current/expired passport
• Birth certificate
• Photocopy of National Registration Card of either parent
• Photocopy of Photocopy of Consent form signed by parent
c. Passport Renewal
• A duly completed Form
• Two (2) recent passport size photographs,
• Current/expired passport
• Photocopy of National Registration Card (NRC)
d. Change of Name on Passport
• Duly completed passport application forms
• Two (2) recent passport size photographs
• Original passport
• Photocopy of National Registration Card (NRC)
• And a copy of any of the following documents:
i. Marriage certificate
ii. Divorce certificate
iii. Death certificate
iv. Deed poll
e. Replacement of lost Passport
In addition to requirements stipulated above, the applicant must:
• Complete passport application forms
• Complete a lost passport description slip/form
• Complete the general affidavit
• Produce a police report
f. Travel document of identity
A non-renewable Travel Document of Identity is issued to Zambian, in cases of emergency or to new born children for one way travel to Zambia.
• Complete application forms,
• Two (2) recent passport size photographs
• Produce Birth Certificate,
• Photocopy of National Registration Card (NRC)
• Police Report, in case of lost passport.
Applicable Fees
• Passport: RM200
• Penalty for lost, stolen, misplaced passport: RM50
• Travel Document of Identity (TDI): RM50
